This course will review the role PM&R can play in post acute care as leaders in development of appropriate pathways, coordination of post acute care, and involvement with administration in developing new care models. It will highlight the appropriateness of physiatrists as leaders in these endeavors. As modern medicine is evolving, the field of rehabilitation medicine has been undergoing multiple changes and facing new challenges.

In order to remain involved and relevant, it is important for rehabilitation physicians to start to reclaim the management and execution of post acute care and involve themselves with this in their institutions. Specific examples will be discussed with reference to the experiences of leaders in PM&R, including looking at new forms of home care, subacute care and acute rehabilitation services. In addition, we will explore the benefits of physiatry directed centralized management of post acute care within a healthcare network and the many benefits which can result.
